Endobronchial pulmonary cryptococcosis and tuberculosis in an immunocompetent host.

چکیده
Pulmonary cryptococcosis presenting as an endobronchial tumour-like growth has rarely been described. We report the case of a male patient with normal immune function who presented with a right upper lobe mass lesion. Bronchoscopy revealed a tumour-like growth that completely occluded the anterior segment of the right upper lobe bronchus. Bronchial biopsy and computed tomography-guided lung biopsy revealed Cryptococcus, and culture of lung biopsy specimen grew Cryptococcus neoformans and Mycobacterium tuberculosis. The patient responded clinically to amphotericin B, fluconazole and anti-tuberculous therapy. However, chest radiographic response was unremarkable. A presentation of pulmonary cryptococcosis and tuberculosis, along with endobronchial tumour-like growth in the same patient, is unusual and has not been previously described.

Primary pulmonary cryptococcosis presenting as endobronchial tumor with left upper lobe collapse.
Primary pulmonary cryptococcosis is difficult to diagnose because the symptoms and radiologic findings are non-specific. Further, pulmonary cryptococcal infections are extremely rare in individuals with normal immunity. This case report describes a case of primary pulmonary cryptococcosis in an immunocompetent individual who presented with left upper lobe collapse due to endobronchial occlusion.
متن کاملMassive mediastinal cryptococcosis in a young immunocompetent patient
Pulmonary cryptococcosis with lymph node involvement is relatively rare in immunocompetent patients. We report a case of pulmonary cryptococcosis with massive mediastinal lymphadenopathy in an immunocompetent young patient. In this report, a 17-year-old boy presented with high-grade fever and persistent cough. Chest X-ray and computed tomography showed massive mediastinal lymphadenopathy. Endob...

Cryptococcosis is a rare opportunistic infection in the immunocompromised host.1,2 Diagnosis of disseminated fungal infections is rarely thought of in immunocompetent hosts. In developing countries where tuberculosis is endemic, Cryptococcosis is often misdiagnosed as tuberculosis.3,4 A 14 year old girl, misdiagnosed as miliary tuberculosis with Koch's abdomen on ATT, presented with seizures. Endobronchial tuberculosis and chest radiography I read, with interest, the article entitled “Clinical and Para-clinical Presentations of Endobronchial Tuberculosis” by Ahmadi Hoseini H. S. et al. (1) published in this journal. I would like to focus on some details about the chest X-ray of patients as elaborated by the authors in the results section. Accordingly, the findings of chest radiograp...

PURPOSE We aimed to investigate clinical and radiologic manifestations of pulmonary cryptococcosis in immunocompetent patients and their outcomes after treatment. MATERIALS AND METHODS We retrospectively reviewed the medical records, initial and follow-up chest computed tomography scans and/or radiographs for initial clinical and radiologic manifestations and outcomes following antifungal tre...
